mRNA Hub initiative by WHO

* mRNA vaccines have played an important role in the fight against the COVID-19 pandemic. However, the vaccine access to the low-and- middle-income countries ( LIMCs ) was inadequate.

* WHO announced in June 2021, the establishment of the Technology Transfer hub to build capacity for production of mRNA vaccines in LIMCs.

* The hub is located in Afrigen, Cape Town, South Africa and works with a network of technology recipients (spokes) in low and middle income countries.

The hub partners:

mRNA platform for non-covid-19 vaccines and therapeutics.

* (mRNA)-based drugs, have proven to be promising therapeutic strategies in immunotherapy.

* The extraordinary advantages associated with mRNA vaccines, such as high potency, relatively low severity of side effects, and the tendency of rapid development, have led to their acceptance in preclinical and clinical trials for various infectious diseases.

* mRNA as a new therapeutic modality was concluded to have the potential to revolutionize medicine.

* BGP has adopted the innovation approach for developing non-covid19 vaccines / therapeutics utilizing mRNA platform technology. (e.g. HIV, Tuberculosis, Ebola or Malaria) .

BGP’s coming steps/needs regarding mRNA project
For R&D activities:

- Set budget for R&D required gaps and proceed in their supply chain cycle

- Equip R&D laboratories with required items according to gap analysis

- Decide along with government, Africa CDC and other entities to identify priorities of candidate vaccines to start R&D activities.

- Proceed with technical training for lab scale.

For pilot & production activities:

- Design for mRNA pilot and production area to be approved by WHO.

- Receive commercial scale package of mRNA COVID-19 to start performing gap analysis.

- Complete training for technology transfer of mRNA COVID-19 platform from South African hub to BGP.

- Proceed with technology transfer process.

Challenges that are facing mRNA hub in LIMCs.

- More funding is required to further expand the reach and impact of mRNA hubs, both in terms of the diversification of vaccines manufactured against different diseases and the increasing number of spokes.

- It could be a while before a South African mRNA vaccine becomes available, as it is expected to take 24-36 months from starting the project.

Right holders sometimes refuse to cooperate, which slows down the development process (but also leaves control over the vaccine with the mRNA hub, which is an advantage).

- Cold chain management especially for the finished product, that can be an opportunity for R&D activities to address this problem which is of critical importance in LIMCs.
